When to refer on to an Exercise Physiologist?If your patient/client is living with: Chronic back, neck, and shoulder pain Musculoskeletal disorders such as knee and hip replacements, or injuries Pre and post surgical interventions (musculoskeletal or metabolic) COPD or asthma Cardiovascular diseases such as hypertension (high blood pressure) or hyperlipidaemia (high cholesterol) Pre-diabetes and diabetes type I and II Pre - obesity and obesity (weight management) Neuromuscular disorders such as multiple sclerosis, Parkinsons' diseases, stroke and cerebral palsy Depression and anxiety Bone related disorders such as arthritis, osteoarthritis, rheumatoid arthritis, osteoporosis and osteopenia Different types of Cancer OR Patient/client is not achieving the recommend 150 minutes of moderate-intensity exercise per week Exercise recommendations can be met through 30-60 minutes of moderate-intensity exercise (five days per week) or 20-60 minutes of vigorous-intensity exercise (three days per week)

How to refer?Complete the webform below, or download a referral form by clicking on the button below Patients must have a GP Management Plan and Team Care Arrangements prepared by their GP. Allied health providers must report back to the referring GP. Alternatively, contact us on 0430 953 122 or just email of the details. Please make sure you include patient diagnosis and any specific requests regarding their treatment. Once a referral is received, we will contact the individual within 1-2 days and arrange an appointment. For insurance-covered programs, we will liaise with insurers regarding approvals. We are always open to discuss treatment options and the best way to help patient/clients just contact us and we can arrange a meeting to discuss your needs.
